2. Your Offer
This is often one of the biggest mistakes that I've seen. Facebook can be used to offer almost anything. However, most people come onto Facebook to become educated or entertained or ideally both.
If you offer a high ticket item without the person knowing you, they will 99.9% of the time not buy from you.
Even if you're offering an ebook or whitepaper or something that's free and informational you need to make sure that the title is enticing. Often this can be done by becoming more niche specific. The perfect guide to accounting is not as valuable as "The Accounting Guide for Marketing Agencies" (at least for Marketing Agencies).
3. Bad Ad Copy (Text)
If the headline doesn't catch the attention of the viewer, then they'll never read the text or click the call to action. You do have a good call to action, right?
There's a whole science and art to copy writing, between those that tell a whole story to those who are short and to the point. Short and to the point is often easier to learn to start, and can sometimes be more effective but the basic key is to clearly show the benefit to the prospect.
Introduce the problem and solution. Cover the value that the prospect gets from what you're offering not the description of your product but rather the benefit.
